These Terms & Conditions govern your use of services provided by SmartOps Health and the clinics we support, including care coordination, appointment scheduling, and text-message communications. By providing your information or using our services, you agree to these terms.
SmartOps Health provides chronic care management, remote patient monitoring, and quality-improvement services to healthcare providers. Communications you receive from us are care-related and are not medical advice. In an emergency, call 911 or your local emergency number.
By opting in, you agree to receive SMS text messages from SmartOps Health and the clinics we support for care-related purposes such as appointment reminders, scheduling links, and preventive-care reminders. You can opt in in person, on a paper or web intake form, or by scanning a clinic QR code.
Message frequency varies. Message and data rates may apply. Reply STOP at any time to unsubscribe, or HELP for assistance. We do not share or sell your mobile number or SMS opt-in data with third parties for marketing. Carriers are not liable for delayed or undelivered messages.
Your use of our services is also governed by our Privacy Policy. Protected health information is handled in accordance with applicable law, including HIPAA where it applies.
We may update these terms from time to time. Continued use of our services after changes means you accept the updated terms.
